Concierge Medicine for Kids

Sometimes referred to as  "Extended Care", Concierge Medicine provides a bespoke medical service to your child. Some of our Doctors are offering this exclusive level of care to a small group of patients. Typically, each Doctor would limit their concierge roster to about 20 patients in order to provide excellent care without being overwhelmed. Concierge care includes telemedicine with a Nurse and same- or next-day home visits (with a per-use fee).

Concierge Care

Some Doctors are opting to offer an "extended care" plan to a small group of patients and their families. This service is necessarily limited as each Doctor has limited capacity and this is offered in addition to the Doctor's typical daily responsibilities.

Extended care offers:

1. All the benefits of the annual block fee (see below), plus...

2. Telephone / text communication directly with our dedicated Nursing team.

3. Access to same- or next-day home visits either by your Doctor, or if your Doctor is unavailable, a surrogate.

4. Preventive care home visits.

Of course, Doctors and Nurses are human. We go on vacation, get sick and sometimes become unavailable for other reasons. The Extended Care plan offers access to the Doctor but patients remain aware that if their Doctor is unavailable, they should make use of available resources, including the Emergency Room or local walk-in clinics.

Extended care fees are set by the individual Physician.

 

Typical fees are $2900 per patient per year. This includes access to same- or next-day home visits. Each home visit would incur a travel fee of $100 per visit.​​

Annual Block Fee

The Ontario Health Insurance Plan (OHIP) pays for most of the services provided in our office, but many services are not insured by OHIP. As these services require time and resources to administer, payment is required. The Annual Fee Program allows you to pay a single annual fee for services not covered by OHIP. This is not mandatory and you may choose to pay for services individually.

 

Services covered by our Annual Fee Program include:

  • Prescription Refills; Lost notes/prescriptions (replacement); Notes/letters on behalf of patients; Sick note/back to school/daycare; Day Care / School notes for medication; Note for orthotics/massage/chiropody/physiotherapy – All notes listed above: $25

  • School/Camp forms $35 (for standard 2-page form)

  • Federal Disability Tax Credit and related forms $65

  • Administration of injections for travel $25

  • Photocopying/Faxing/Email reports requested by patient $35

  • Travel Cancellation Insurance Form $35

  • TB Skin Test (3rd party request) $42

 

Services NOT COVERED by the Annual Fee Program include:

  • Missed Scheduled Check-up (i.e. wellness) Appointment $60

  • Missed Counselling Appointment $60

  • Physician Review of Records $45 per 15mins

  • Insurance Forms and Legal Reports $200

We service downtown and "old" Toronto, south of the 401 between Dufferin and Bayview, with few exceptions.
