Annual Block Fee
and
Extended Services
The annual block fee is optional and covers services that OHIP does not pay for.
Also, read below about our exclusive extended care program, limited to 20 patients per Doctor, which includes telemedicine and same- or next-day home visits (with a per-use fee).
Annual Block Fee
The Ontario Health Insurance Plan (OHIP) pays for most of the services provided in our office, but many services are not insured by OHIP. As these services require time and resources to administer, payment is required. The Annual Fee Program allows you to pay a single annual fee for services not covered by OHIP. This is not mandatory and you may choose to pay for services individually.
Services covered by our Annual Fee Program include:

Prescription Refills; Lost notes/prescriptions (replacement); Notes/letters on behalf of patients; Sick note/back to school/daycare; Day Care / School notes for medication; Note for orthotics/massage/chiropody/physiotherapy – All notes listed above: $25

Pediatric Extended Care
Some Doctors are opting to offer an "extended care" plan to a small group of patients and their families.
​
Extended care offers:
1. All the benefits of the annual block fee, plus...
2. Telephone / text communication directly with the Physician or the Physician's surrogate (either an alternate Doctor or a Nurse).
3. Access to same- or next-day home visits either by your Doctor, or if your Doctor is unavailable, a surrogate.

Of course, Doctors are only human. Even Doctors go on vacation, get sick and sometimes become unavailable for other reasons. The Extended Care plan offers access to the Doctor but patients remain aware that if their Doctor is unavailable, they should make use of available resources, including the Emergency Room or local walk-in clinics.

Extended care fees are set by the individual Physician.
Typical fees are $1800 per patient. This includes access to same- or next-day home visits. Typically, the travel fee (not covered by OHIP) would be waived for the first 5 home visits. Subsequent visits would be accompanied by a travel fee of $100 per visit.​​
